HHA An important member of the interdisciplinary team who works under the supervision of a Registered Nurse (RN)
and performs various services for a patient to promote improved health. Responsible for observing the patient,
reporting these observations to the RN case manager and documenting observations and care performed. Will
be required to follow the aide plan of care as assigned by the RN case manager. This plan of care will promote
quality, continuity and safety.
The following statements describe the principal activities of this position identified and shall not be construed
as a detailed exposition of all the duties that may be inherent in this position.
• Performs personal care needs as assigned by a RN;
• If unable to perform a certain task will document and report this information to the RN case manager and
clinical director immediately;
• Provides personal care which will include: bathing, oral hygiene, shampoos and changing bed linen as often
as assigned;
• Assists in dressing and undressing patients as assigned;
• Takes and records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when
ordered with appropriate completed/demonstrated skills competency;
• Provides patient and family with positive communication techniques, within realm of ethical and respectful
care, including confidentiality;
• Meets safety needs of the patient;
• Educated and knowledgeable with use of ordered equipment: foot stools, side rails, O2, etc.;
• If unfamiliar with equipment in the home will document and report this information to the RN case manager
and director of nursing immediately;
• Provides proper care and observation of patient's skin to prevent breakdown of tissue over bony
prominence;
• Reports any concerns with skin integrity to the case manager immediately following visit;
• Assesses and reports on patient's condition and significant changes to the RN case manager; • Assists in ambulation and exercise as assigned in the patient's plan of care; • Provides normal range of motion and transfers/positioning as directed; • Offers and assists with bedpans and urinals; • Keeps patient's living area clean and orderly, as assigned; • Adheres to the organization's documentation and care procedures and standards of personal and professional conduct; • Respectful of patient and family/caregiver environment and patient's personal needs; • Complies with Compliance Plan; • Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient's care; • As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position. Must be minimum 18 years of age per state guidelines
• Must possess an active Certified Nursing Assistant (C.NA) licensure for the State of practice; • Must possess a home health aide certificate issued by the licensing agency; • Must possess and maintain a current CPR certification.
